Player substitutions can be completed only:

after a goal and before the substituting team has signaled readiness; or

8.A.2

to replace injured players, replace players with illegal equipment (7.D.3), or to resolve an incorrect personnel violation (9.B.3.a). In this case, the opposing team may substitute a like number of, or fewer, players.
